Piedmont was leading the way. Now, the rules could change for everyone. After more than a year of pandemic, the hand is finally being put in place Covid home care protocol to be adopted for patients at home, which could undergo major changes and become homogeneous throughout Italy.

After months of discussions, failed attempts and “misdirection”, even involuntary, Friday 9 April the Senate approved, with 212 votes in favor, and only 2 against and 2 abstentions, the agenda signed by all parliamentary groups for the Government to take action for the establishment of a single national protocol for the home management of Covid patients.

The text, not a foregone conclusion of a agreement between all political forces, following the withdrawal of the previous motions presented by M5s and Lega, opens the way to treatments used for over a year by hundreds of doctors throughout Italy, discordant with the guidelines of AIFA and the Ministry of Health, which to date still recommend in first days of symptoms potentially attributable to the Coronavirus “Tachipirina and watchful waiting”.

Why we need to act now: what to do with the first Covid symptoms

For some time we at QuiFinanza have turned the spotlight on so-called “alternative” therapies, which for months have been neglected by many media, doctors and experts. Now, the clinical evidence has clearly shown that it is in the very first days of onset of symptoms, even very mild ones, even without fever, that action must be taken, “Anticipating” the virus in a certain sense, which could rapidly degenerate if approached in the wrong way.

As pointed out for some time by prof. Giuseppe Remuzzi in Bergamo (here you can find the in-depth analysis of QuiFinanza) and the hundreds of doctors who offer their support, completely free of charge, in Facebook group # TERAPIADOMICILIARECOVID19 in each Region, it is necessary to immediately treat the potentially Covid symptoms, well before the tampon, like any other inflammatory disease, and therefore prescribing the so-called NSAIDs, such as aspirin and Aulin for example, able to inhibit any inflammatory process that could trigger, up to , in the most serious cases, to the cytokine storm that hits the body leading to hospitalization, and in some cases even to death (here you will find all the benefits of aspirin, also with regard to Covid).

NSAIDs are preferred over paracetamol (Tachipirina is the most famous paracetamol, but there are others), because the latter not only has a low anti-inflammatory activity but, according to some experts, decreases the stocks of glutathione, a substance that acts as an antioxidant. Glutathione deficiency could lead to further worsening of the damage caused by the inflammatory response, which occurs during Covid infection.

The strange case of Piedmont

We said of the Piedmont, the first Region in Italy to embrace, at least on paper, the new protocol. A case, however, emblematic. Because despite being the first to update treatments with the new indications, very few general practitioners actually apply them, and the outcome of this lack of adjustment is evident in the number of admissions.

Piedmont, in fact, ranks at first place among the Regions for the number of hospitalizations compared to the total of positive ascertained. The data from Agenas fix the national average at 6%, while Piedmont is at 13.6, only the Autonomous Province of Bolzano is worse at 15.7.

It is true, as the Regional Health Councilor Luigi Icardi pointed out, that Piedmont has one of the oldest population, but above all “there are still problems in territorial medicine. The data tell us that where the protocol for home care is applied in a widespread manner, hospitalizations drop “. In the capital Turin, for example, the protocol is struggling to take off: it is difficult to understand the reasons, since it would avoid hundreds of hospitalizations.

Antonio Barillà, regional secretary of the SMI, one of the unions of family doctors, explained that on the protocol for home care, both for the part that provides for the activation of the Usca and for the one that contemplates integrated home care, there are a series of problems that been reported to the Region. “Problems about the actual availability and ability to provide all the necessary services, starting from home blood tests, x-rays and other tests.

The Usca, where active, struggle, a lot. 15 thousand requests in Piedmont alone in March, yet integrated home assistance, with visits to the doctor and nursing assistance, which sees the doctor recognize 75 euros for each practice and 27 euros for each home visit, in hiding.

What changes for anti-Covid treatments after approval in the Senate

Now, the approval in the Senate could really represent a change of course for thousands of Italians who until now have seen themselves neglected by their general practitioners or not treated in the correct way, with a excess hospital admissions which has led to a very heavy overload for the health system, both in medical areas and in intensive care. A battle won primarily by all the doctors of the Covid-19 Home Care Committee created by the lawyer Erich Grimaldi, who in this year have treated patients at home, demonstrating the effectiveness of the therapeutic scheme and of a teamwork never achieved before in Italy.

Grimaldi managed to create a coordinated work group made up of thousands of professionals with a single goal: to help patients, treat them in a timely and adequate manner, and avoid them being hospitalized. All absolutely free.

In its official press release, the Committee wanted to express thanks also to those who, in politics, “listened to our requests and were personally active and committed to achieving the result, especially Senator Massimiliano Romeo, leader of the Lega , and the Undersecretary of Health Pierpaolo Sileri ”.

The next step is already operational: sit down at a table to define the redefinition of the early home care protocol as soon as possible. “We are proud to have achieved this result, net of a year of relentless commitment by everyone”, commented the chairman of the Grimaldi Committee, “I have always been certain of the importance of the work we have done and continue to do and this result confirms it ”.

At the working table, Grimaldi hopes that Professor Luigi Cavanna and Dr. Andrea Mangiagalli, members of the scientific council of the Committee, will also sit as an expression of the two realities more necessary than ever in this pandemic: hospital and territorial medicine, “which have collaborated in an excellent way to give life to the therapeutic scheme that has healed thousands of people in our country ”.
